As a Health Care Plan, Capstone has been in existence for over 25 years. We are contracted with the Division of Developmental Disabilities. Capstone is the only Health Care Plan in Arizona dedicated exclusively to those with Developmental Disabilities. You are our sole focus and purpose. Capstone Health Plan is a non-profit corporation; all revenue is dedicated to serving our Members.
We offer a Provider Network that provides convenience and expertise in the 5 Northern Arizona Counties we serve. We take pride in our Provider Network of physicians, practitioners and other health care professionals. These dedicated individuals with whom we partner are pleased to serve you.
Capstone Health Plan exists to support you in getting the medical care you need. Our staff is available for you and strives to provide personal attention.
