About Us

Capstone Health Plan, Inc. is a Flagstaff, Arizona-based non-profit health care plan that is contracted with the Arizona Department of Economic Security's Division of Developmental Disabilities (DES/DDD). We provide acute health care services to eligible Arizona Long Term Care (ALTCS) Members. We manage the medical benefits and services that you are entitled to as a Member.

We partner with healthcare professionals such as doctors, hospitals, behavioral and rehabilitative agencies in five rural Northern Arizona counties:

As a DES/DDD Health Plan, we follow the Arizona Health Care Cost Containment System (AHCCCS) rules for services. We are the only Health Plan in Arizona that is exclusive to those with Developmental Disabilities. It is our goal to work with our Members and their families to ensure your health care needs are met.
