Capstone Health Plan has been providing services to people with developmental disabilities in Northern Arizona since 1981. Capstone understands the unique needs of those with disabilities and helps them navigate what can be a confusing selection of agencies and services. Capstone networks on an ongoing basis with your Primary Care Providers, DDD Support Coordinators, Therapists, Regional Behavioral Health Authority (RBHA) and Children's Rehabilitative Services (CRS) clinics to help provide you with the best possible service.
