Capstone Health Plan
Capstone Health Plan has been providing health care services in northern Arizona since 1981. We are contracted with the Arizona Department of Economic Security-Division of Developmental Disabilities to provide services to individuals with disabilities who have qualified for the Arizona Long Term Care (ALTCS) program. Capstone is the only Health Care Plan in Arizona dedicated exclusively to serving people with Developmental Disabilities. Capstone Health Plan is a non-profit corporation that dedicates all revenue to providing quality health care services to our Members.
Capstone offers a comprehensive network of health care providers. We take pride in our network of physicians, practitioners, dentists, hospitals, pharmacies and other health care professionals. In addition to an extensive network throughout the state of Arizona, Capstone has health care providers in all bordering communities of California, Nevada, Utah and New Mexico.
