Pharmacy
Multiple pharmacies, both chain and independent, conveniently located near you.
DME
Durable Medical Equipment and Medical Supplies such as oxygen, wheelchairs, nutritional feeding supplies, etc. can be provided by our contracted providers in your area.
Transportation
Medically necessary transportation can be provided for Capstone covered services and CRS appointments. Capstone offers a travel reimbursement program for our Members who use their own transportation that minimizes your out of pocket costs.
Dental
Routine dental services are available to Members under the age of 21 and do not need a referral. Emergency dental services are provided to Members of all ages.
Vision
Capstone provides one eye exam and a pair of eye glasses once a year for Members under the age of 21. Capstone also provides repair or replacement of eye glasses once a year. Eye exams for diabetics are covered once a year regardless of the Member's age.
Hospitals
Major hospitals throughout Northern Arizona to serve our Members. We also offer services in the metropolitan areas of the state including Phoenix Children's Hospital, St. Joseph's Hospital and University Medical Center in Tucson.
Children's Rehabilitative Services & Behavioral Health
Capstone Health Plan works closely with other health programs such as Children's Rehabilitative Services (CRS) and the Regional Behavioral Health Authority (RBHA). These agencies provide specialty services for enrolled Members with chronic and complex medical needs or behavioral health needs. Our coordination of care between health care programs helps to ensure our Members get the quality of health care they need in a timely manner.
