Children's Rehabilitative Services
Children's Rehabilitative Services (CRS) is a program administered by AHCCCS (United Healthcare APIPA) that is designed to provide specialty medical and surgical care to children up to age 21. The CRS program provides a comprehensive, multi-disciplinary approach to care for children with special needs. CRS providers are experienced pediatric practitioners. CRS covered services include visits to a specialist, inpatient care, surgery for a CRS condition, therapy, prescription drugs, DME, home health care and social and educational services, as well as periodic follow-up.
CRS does not provide primary care or care for any conditions that are not CRS conditions. Capstone is responsible for covering these services.

CRS Eligibility
All Capstone members are financially eligible for CRS. Medical eligibility is determined if:
- Child has a CRS covered condition as defined in the CRS Policy and Procedure Manual, such as cerebral palsy, seizures, spina bifida, scoliosis, cleft lip and palate, etc.
- Child requires comprehensive multi-disciplinary care.
- Child has a reasonable potential for habilitation.
Applying for CRS

It is best to work with your PCP or Capstone Health Plan to complete an initial application for CRS. This is because CRS requires very specific medical documentation of eligible conditions, which typically must include PCP and specialist notes.
Per AHCCCS rules Members who have Capstone as their only insurance must pursue enrollment in CRS. Please contact the Capstone Health Services Department for assistance in applying for CRS.
