Capstone Health Plan maintains an extensive network of primary care and specialty care providers throughout Northern Arizona, Phoenix and Tucson.

It is our goal to provide Members with a choice of qualified professionals to support and deliver exceptional services to those with developmental disabilities.

Good communication is the key to the successful partnership we share with our Provider Network. Our Provider Relations Department is available Monday thruough Friday, 8:00am to 5:00pm for your office staff, physicians, practitioners, dentists and other health care professionals to provide assistance, answer questions and resolve issues. If you are a primary care provider, a Capstone representative will visit you and your office staff on a regular basis to share important information and assist you with administrative or operational concerns.
