Provider Contracting
The Provider Relations Department receives many requests from potential providers to contract with Capstone Health Plan. Some of the factors considered in any potential contracting decision are:
- Geographic network needs based on AHCCCS requirements
- Existing provider coverage in the targeted area
- Membership requirements for a particular specialty
- Member accessibility

Once we have determined your intent to contract as a Capstone provider we will initiate the credentialing process. Capstone Health Plan credentials providers according to nationally recognized standards that serve as the model for minimum credentialing requirements. We subcontract with Greater Arizona Central Credentialing to perform our credentialing verification process.
Not all providers must be contracted and credentialed with Capstone Health Plan to provide services to our Members. Contact our Provider Relations Department for more information.
