Is Capstone an AHCCCS Plan?
Capstone subcontracts with the Department of Economic Security's Division of Developmental Disabilities (DES/DDD) to manage the health care services for Arizona Long Term Care Members. Capstone adheres to all AHCCCS guidelines and rules.
What areas of Arizona does Capstone provide health care services?
Capstone's services areas include Coconino, Yavapai, Apache, Navajo and Mohave Counties. In addition, we have providers in Phoenix and Tucson, and in the border towns of Utah, California, Nevada and New Mexico.
How do I become a participating provider with Capstone Health Plan?
